People Encouraging People (PEP) accepts referrals for a broad range of high-quality, person-centered services—including Assertive Community Treatment, Forensic Community Treatment, Residential and Psychiatric Rehabilitation, Health Homes, Transitional Age Youth services, Deaf Unity, Supported Living, Homeless Outreach, Group Practice, Intensive Case Management, and Housing—designed to promote independence and community integration.

If you are a provider, case manager, hospital, community partner—or a family member, caregiver, individual seeking services, or anyone wishing to make a referral—please download the appropriate referral form below and follow the submission instructions provided.

How to Submit a Referral

For most PEP services:
  • Download the PEP Referral Form.
  • Complete all required information.
  • Include any requested supporting documentation.
  • Submit the completed referral according to the instructions provided on the form.

For Residential Rehabilitation Program (RRP) referrals:
  • Complete the Statewide RRP Application.
  • Compile and attach all required supporting documentation.
  • Submit the application packet to the CSA or LBHA in the applicant’s jurisdiction of origin.
  • The CSA or LBHA will screen the referral for RRP eligibility and determine whether additional services are needed.
  • If additional services are needed, the CSA or LBHA may direct the referral source or applicant to Carelon.
  • Within two working days, the CSA or LBHA reviews the application and, if appropriate, refers the applicant to an RRP with an available bed.
  • The CSA or LBHA authorizes an assessment.
  • Within five working days of receiving the referral/assessment authorization, the RRP evaluates the applicant and issues a disposition.
  • If accepted, the CSA or LBHA signs a Certificate of Determination (COD) to approve placement and level of care.
