Assertive Community Treatment

Assertive Community Treatment (ACT) is an intensive, team-based mental health service for individuals with serious and persistent mental illness who need comprehensive, community-based care. Services are delivered by a multidisciplinary team—including a program manager, psychiatrist or psychiatric mental health nurse practitioner, nurses, therapist, housing specialist, vocational specialist, and peer specialists—who provide individualized support.

The ACT team meets people where they are—in their homes, workplaces, and communities—focusing on symptom management, daily living skills, housing stability, access to care, and strengthening community connections.

Forensic Assertive Community Treatment

Forensic Assertive Community Treatment (FACT) is an intensive, team-based service supporting justice-involved individuals with serious mental illness who need comprehensive, community-based care. A multidisciplinary team—including a psychiatrist, nurses, therapist, housing and vocational specialists, and peer support—provides individualized services focused on reducing recidivism and incarceration. Rather than office-based care, services are delivered in homes and communities. Support includes symptom management, daily living skills, housing stability, medical care, and strengthening natural supports. FACT promotes recovery, stability, and independence through consistent, person-centered care delivered when and where it is needed most.

Our Health Home services deliver coordinated, whole-person care for individuals with complex behavioral health and medical needs. A dedicated team integrates primary care, mental health services, and community supports, providing care coordination, health promotion, transitional care, and connections to essential resources. Using a client-centered, outcomes-driven approach, we help individuals manage chronic conditions, reduce hospitalizations, and improve overall wellness. We partner with providers and community organizations to develop personalized care plans aligned with each person’s goals. Through prevention, education, and ongoing support, we empower clients to take an active role in their health and achieve lasting stability and improved quality of life.

Residential Rehabiliation Program

Our Residential Rehabilitation Program (RRP) provides structured, community-based housing and supportive services for adults living with serious mental health conditions. In a safe, home-like setting, residents receive guidance while building skills for greater independence. Working alongside trained staff, individuals develop practical abilities such as medication management, budgeting, cooking, and maintaining daily routines. Services also emphasize symptom management, social development, and connections to employment, education, healthcare, and community resources. With consistent support and crisis assistance, RRP fosters stability, promotes independence, and prepares individuals to transition successfully into independent housing and fuller participation in community life.

Transitional Age Youth

Our Transitional Age Youth (TAY) program provides structured, community-based housing and supportive services for young adults ages 18 to 25 as they transition from adolescence to adulthood while managing behavioral health needs. The program helps participants build independence, life skills, and resilience, offering guidance, mentorship, and access to services that support educational, vocational, and personal growth. Participants receive individualized support that includes counseling, skill-building workshops, peer mentoring, and connection to community resources such as housing, employment, education, and healthcare. The program emphasizes empowerment, self-advocacy, and long-term stability, helping young adults successfully transition into adulthood with confidence and support.

Deaf Unity

Our Deaf Unity program provides structured, community-based support for adults who are deaf or hard of hearing and living with serious behavioral health conditions. Services are delivered on-site and in the community, fostering a safe, inclusive environment where individuals pursue goals and build stability. Culturally competent staff understand the communication and lived experiences of the Deaf community, offering individualized, goal-oriented support. Qualified interpreters ensure access to behavioral and physical healthcare. The program reduces isolation and communication barriers while strengthening daily functioning. Through skill development, education, and connection, participants gain independence and achieve meaningful, fulfilling lives grounded in long-term recovery.

Individualized in-home support services are provided in the client’s home. Support may include assistance with mediation management, symptom’s monitoring and management, home management, self-care, community integration, money management and accompaniment to medical or other appointments.

Our Psychiatric Rehabilitation Program (PRP) offers community-based services that help individuals with mental health conditions build the skills and supports needed to thrive. Our group practice provides comprehensive, client-centered behavioral health care by integrating psychiatric and therapeutic services. Our psychiatric providers offer thorough evaluations, accurate diagnoses, and ongoing medication management for conditions such as anxiety, depression, mood disorders, and trauma-related concerns. Our licensed therapists provide evidence-based counseling—including CBT, trauma-informed care, and mindfulness techniques—to help clients build coping skills and improve well-being. Working collaboratively, our team ensures clear communication, continuity of care, and personalized treatment that supports lasting outcomes.

The Baltimore Comprehensive Overdose Response to End the Epidemic (BCORE), a city-wide, multi-disciplinary collaborative, engages people at key points of crisis response and emergency care to provide them with access to medication for opioid use disorder (MOUD) and connect them to long-term supportive care and services.

Affordable Housing

We have been providing affordable housing for individuals with limited incomes and disabilities for over 30 years. Today, the organization owns and operates a portfolio of 75 homes, including units supported through HUD Section 8 Housing Choice Voucher Program and HUD Section 811 Supportive Housing for Persons with Disabilities, as well as homes leased at Fair Market Rent.

We also operates nine subsidiary companies – Accension, Mt. Washington, Clovil, Hampton Falls, Overly, Greater Hamilton, Dartmouth, and Plymouth – offering housing in a variety of community-based settings throughout Baltimore City and County. Within this portfolio, 13 homes and 11 townhomes support the Residential Rehabilitation Program, serving both intensive and general-level clients. These residences provide structured, supportive housing designed to promote stability, independence, and community integration.
